Insurance Cost

In Arizona, Chronic Special Needs health insurance is available for seniors, and other Medicare eligible individuals, who have been diagnosed with diabetes by a licensed medical practitioner. Subject to the specific special needs plans that are available in your county, the cost can vary from no cost (for AHCCCS Medicaid eligible seniors) to low cost (for seniors who have too much income to qualify for AHCCCS Medicaid but are nonetheless living on a limited income).

For seniors who have both Medicare and AHCCCS Medicaid, most of the costs of participating in a Diabetic Medicare Chronic Special Needs plan will be covered by federal and state governments. For seniors who do not qualify for AHCCCS Medicaid, the cost will depend on the Medicare Advantage plan selected.


Insurance Benefits

Diabetes Chronic Special Needs health insurance plans provide all of the services that the federal government requires for Medicare Advantage plans. Many plans also provide additional coverage that may include zero monthly premiums and low or no cost primary physician and specialist visits. Some plans also provide coverage for hearing services, vision, dental, mental health, skilled nursing, physical therapy, transportation, pharmaceuticals, home health services, over the counter health products, routine foot care and wellness programs. And, some Medicare Chronic Special Needs plans also provide a care coordinator who helps plan members get the right care, at the right time, from the right providers.
